INSURANCE INFORMATION

  • I am an out of network provider with insurance companies. I provide a super-bill for reimbursement. Please contact your insurance company to inquire about your coverage and reimbursement rate. 

  • Payment is due at the time of service. Credit card and Zelle payment are accepted.
